Women who experience menopause at a younger age may face faster cognitive decline, greater accumulation of brain abnormalities, and an earlier diagnosis of Alzheimer's disease decades later, according to a major long-term study.
Researchers analyzed 2,603 older women participating in the Religious Orders Study and Rush Memory and Aging Project, with follow-up extending up to 18 years. The research included investigators from the University of California, San Francisco Weill Institute for Neurosciences, Rush University Medical Center, and the University of Michigan.

Earlier Menopause Linked to Faster Cognitive Decline
Participants underwent annual standardized assessments measuring overall cognition and individual abilities, including episodic and working memory. Clinicians also conducted yearly evaluations to determine when Alzheimer's disease developed.
Earlier menopause was associated with faster deterioration in global cognition and episodic memory, which involves remembering personal experiences and events. Importantly, women reaching menopause earlier were also diagnosed with Alzheimer's sooner.
Researchers calculated that menopause occurring 10 years earlier corresponded to approximately 1.8 fewer Alzheimer's-free years around the cohort's average Alzheimer's diagnosis age of 88.4 years. This

Brain Scans Reveal Structural Differences
The investigation went beyond cognitive testing. Among the participants, 774 underwent repeated high-resolution 3-Tesla MRI brain scans, while 1,287 women who died during follow-up underwent neuropathological examination at autopsy.
One of the strongest findings involved white matter hyperintensities, abnormalities visible on MRI that reflect changes in the brain's white matter. Among women experiencing spontaneous, or natural, menopause, younger menopause age was associated with substantially faster accumulation of these abnormalities.
A woman who experienced natural menopause five years earlier accumulated approximately 15 percent more white matter hyperintensity volume over a decade. The association became stronger with advancing age but was not observed in women with surgical menopause. Associations between menopause timing and total brain volume loss were comparatively modest.
Hormonal Timing Could Identify Risk Earlier
Menopause involves the loss of cyclic ovarian hormone production, particularly estrogen. Estrogens influence synaptic plasticity, myelination, mitochondrial activity, and neuroimmune function, providing biological pathways that could help explain the relationship between reproductive aging and later brain health.
The researchers stressed that menopause age cou
ld become a useful midlife marker for identifying women at increased risk long before cognitive or structural brain changes become apparent.
Conclusion
The findings indicate that earlier menopause is associated with lasting differences in cognitive and structural brain aging, suggesting menopause timing could help guide future risk assessment and targeted preventive strategies before neurological changes become clinically evident.
