A new Portuguese study has revealed that memory and thinking problems in fibromyalgia are far more common and serious than many people realize. Researchers found that more than seven out of ten women living with fibromyalgia showed clear signs of cognitive dysfunction, often described by patients as brain fog. Who conducted the research
The study was carried out by researchers from the Rheumatology Department of Unidade Local de Saúde da Região de Aveiro, the Aveiro Rheumatology Research Centre, the Rheumatology Unit of Unidade Local de Saúde Tondela Viseu, and the Physical and Rehabilitation Medicine Department of Unidade Local de Saúde da Região de Aveiro in Portugal.
 
How the study was done
Scientists compared 47 women diagnosed with fibromyalgia to 19 healthy women of similar age and background. They used a simple but reliable test called the Montreal Cognitive Assessment to measure memory attention language and problem-solving skills. A score below 26 suggested cognitive dysfunction. The team also measured pain fatigue sleep quality anxiety depression and overall daily functioning.
 
Striking differences between patients and healthy people
The results were dramatic. About 72 percent of fibromyalgia patients had cognitive problems compared to just 5 percent of healthy participants. Those with fibromyalgia also reported much higher pain levels severe fatigue poor sleep and emotional distress. Many struggled with daily activities work responsibilities and social life.
 
What makes brain fog worse
The researchers discovered that cognitive problems were closely linked to how badly the disease affected daily life. Higher pain intensity and depressive symptoms were strongly connected to lower thinking and memory scores. Patients with worse disease impact scores were significantly more likely to experience cognitive dysfunction even after accounting for medications and other factors.
 
Why this matters for patients
These findings confirm that brain fog is not imagined or exaggerated. It is a measurable and common part of fibromyalgia that deserves proper attention. The study suggests that managing pain and depression may also help protect thinking abilities. Doctors may need to routinely screen fibromyalgia patients for cognitive problems so help can begin earlier.
 
What the researchers concluded
The study clearly shows that cognitive dysfunction is widespread in fibromyalgia and closely tied to pain depression and overall disease burden. Addressing both physical and emotional symptoms together may improve mental clarity and qual ity of life for many patients.
