Study Finds Lasting Eye Changes in Recovered Patients
A new study from researchers at the Second Affiliated Hospital of Fujian Medical University in Quanzhou, China, has found that people who recovered from COVID-19 may continue to experience subtle but potentially important changes in the transparent front surface of the eye. The findings suggest that the virus could have longer-term effects on the cornea, even after recovery, raising concerns for future eye health and certain eye surgeries.

Corneal Cells Show Signs of Damage After COVID-19
The researchers conducted a cross-sectional study involving 760 participants who underwent corneal endothelial examinations between August 2022 and May 2023. Among them, 378 individuals had recovered from COVID-19, while 382 had no history of SARS-CoV-2 infection.
 
The team measured several indicators of corneal endothelial health, including endothelial cell density (ECD), average cell area (ACA), standard deviation of cell area (SD-Area), and the percentage of hexagonal cells (HEX). Corneal endothelial cells are vital because they keep the cornea clear by regulating fluid levels. Unlike many other cells in the body, these cells do not regenerate effectively once lost.

Patients who had recovered from COVID-19 showed a significant reduction in endothelial cell density compared to those who had never been infected. They also had fewer normally shaped hexagonal cells, while average cell size, variation in cell size, and maximum cell area were all significantly increased. These changes indicate that the remaining cells had enlarged and become more irregular to compensate for cell loss.
 
Age and COVID-19 Both Increased Risk
To ensure the findings were reliable, the researchers analyzed factors such as age, sex, and medical history. Multiple linear regression showed that both a history of COVID-19 infection and older age independently contributed to lower endothelial cell density and abnormal cell morphology. This means the observed eye changes were not simply explained by aging or other health conditions.

The researchers believe these structural changes could reduce the cornea's ability to tolerate stress, particularly during procedures such as cataract surgery or corneal transplantation. Although most recovered patients may not immediately notice vision problems, reduced endothelial reserve could increase the risk of complications later in life.
 
Findings Highlight Need for Long-Term Monitoring
The researchers emphasized that further prospective studies with eye measurements taken before infection are needed to confirm whether COVID-19 directly causes these changes. They also hope future research will help develop evidence-based guidelines to better protect the corneas of individuals with a previous COVID-19 infection before undergoing eye surgery.
 
Conclusion
The findings provide compelling evidence that COVID-19 recovery may leave behind measurable and lasting changes in the cornea's protective endothelial cell layer. Although additional research is needed to establish direct causation, the results suggest that recovered patients, especially older adults, may benefit from careful eye evaluations and corneal monitoring before undergoing ophthalmic procedures.
