Medical News: As the world continues to live with COVID-19, scientists have completed one of the largest reviews ever conducted on medications used outside hospitals to treat the infection. The findings paint a sobering picture: despite years of research and widespread interest in repurposed medicines, very few oral or inhaled drugs consistently reduced the risk of severe illness.


Massive review of 124 clinical trials finds that most widely promoted outpatient COVID-19 drugs offered little consistent protection against severe illness
 
The research team came from the Upstream Lab and MAP Centre for Urban Health Solutions at St. Michael's Hospital, Unity Health Toronto, the University of Toronto, the University of British Columbia, the University of Manitoba, Memorial University of Newfoundland, and an independent information specialist based in Ottawa, Canada.
 
Largest Review of Outpatient COVID-19 Treatments
The researchers analyzed data from 124 randomized clinical trials involving 137 different treatments conducted across 35 countries. Their goal was to determine which medicines actually helped adults with COVID-19 who were treated at home rather than in hospitals.
 
The review focused on oral medications and inhaled therapies, comparing antiviral drugs with medicines that had originally been developed for other diseases but were repurposed during the pandemic.
 
More than half of the trials were judged to have a low risk of bias, making the overall findings more reliable than many earlier reviews.
 
Most Repurposed Drugs Showed Little or No Benefit
Many drugs that generated enormous public attention during the pandemic failed to demonstrate consistent clinical benefits.
 
Hydroxychloroquine, ivermectin, fluvoxamine, colchicine, azithromycin, doxycycline, camostat, anticoagulants, and several other repurposed medicines generally did not reduce hospital admissions, intensive care admissions, or deaths in a consistent manner.
 
Although some smaller studies initially suggested positive effects, those benefits usually disappeared when larger and better-designed clinical trials were completed. This indicates that early encouraging results often could not be reproduced in broader patient populations.
 
A Few Treatments Produced Limited Improvements
Not every therapy failed completely. Inhaled budesonide consistently helped some patients recover more quickly from COVID-19 symptoms, particularly in early disease. Metformin also showed modest benefits in selected studies, including reductions in emergency department visits, although these findings were not consistent across every trial.
 
Molnupiravir emerged as the antiviral with the strongest evidence among outpatient treatments. The drug shortened recovery time by several days in multiple clinical trials and reduced hospitalization among unvaccinated patients considered at high risk for severe disease. However, the drug is also known to cause rapid mutation of the virus.
 
Also, its advantages became much less obvious when res earchers included studies involving mostly vaccinated populations. The protective effect against hospitalization was substantially weakened, suggesting vaccination status and overall patient risk strongly influence how well the drug performs.
 
Why Later Studies Produced Different Results
This Medical News report highlights an important reason why many early COVID-19 drug studies now appear less impressive.
 
The earliest trials were conducted before vaccines became widely available, when patients faced a much higher risk of hospitalization and death. Later studies enrolled mostly vaccinated individuals who had lower risks of severe illness and often possessed immunity from previous infections.
 
As newer variants emerged and standard medical care improved, the ability of many drugs to demonstrate meaningful clinical benefits naturally declined.

The researchers emphasized that treatment effectiveness cannot be interpreted without considering vaccination status, viral variants, patient risk factors, and how early treatment begins after symptoms appear.
 
Safety Findings Offer Reassurance
The review also examined drug safety. Most medications produced side effects that matched their known safety profiles. Hydroxychloroquine mainly caused digestive problems, while colchicine frequently produced gastrointestinal symptoms. Fluvoxamine caused nausea, indigestion, and insomnia in some participants. Molnupiravir, ivermectin, inhaled budesonide, and metformin were generally well tolerated, and investigators found no unexpected safety concerns across the trials.
 
Conclusions
The comprehensive review demonstrates that most repurposed COVID-19 medications that generated global interest ultimately failed to provide reliable protection against hospitalization, intensive care admission, or death in non-hospitalized patients. While inhaled budesonide, metformin, and particularly molnupiravir showed benefits in selected settings, their effectiveness depended heavily on patient characteristics, especially vaccination status and baseline risk. The findings reinforce the importance of relying on high-quality clinical evidence rather than early promising reports when making treatment recommendations for future viral outbreaks.
